Payment can be made by cheque, payable to: "RANZCO NZ Branch Meeting 2009", or credit card - MasterCard and Visa (American Express and Diners are not accepted). All prices quoted are in New Zealand Dollars and inclusive of GST.

Please note that payment must accompany your Registration Form, your place will not be confirmed until payment has been received. Invoiced registrations will be cancelled if payment is not received within 10 working days of invoice.

Specialists, Ophthalmic Associates Full Registration includes:
Attendance at all conference sessions, delegate conference materials, morning and afternoon teas, lunches, welcome cocktail function on Thursday night, and combined conference dinner on Saturday night.

Orthoptists Full Registration includes:
Attendance at all conference sessions, delegate conference material, morning and afternoon teas, lunches, and Orthoptists' social functions. Attendance at the optional Sunday morning Alcon Workshop on Biometry - Optimising cataract outcomes (Biometry, calculation formulae and IOL selection) and Pharmaceutical update. The full fee does not include the combined conference dinner.

Nurses Full Registration includes:
Attendance at all conference sessions, delegate conference materials, morning and afternnon teas, lunches, the Alcon Workshop on Biometry - Optimising cataract outcomes (Biometry, calculation formulae and IOL selection) and Pharmaceutical update, welcome cocktail function on Thursday night, Nurses social function on Friday night, and combined conference dinner on Saturday night. The full fee does not include the optional Nurses dinner on Friday night.

Day registration for all groups includes:
Conference session admission on the day of registration, delegate conference materials, morning and afternoon tea and lunch on day of registration. The day registration does not include admission to any of the social functions.

Acknowledgement
All registrations will be acknowledged in writing. If you do not receive an acknowledgement please contact the team at Six Hats.

Registration desk location and opening times
Your registration pack will be available for collection from the Six Hats registration desk at the Rutherford Hotel Nelson. The Six Hats registration desk will be open at 7.30am Thursday 14 May and remain open throughout the conference.

Special Requirements
Please give details on your registration form of any special diet or disablity assistance required.

Cancellation and Refund Policy
All cancellations must be made in writing to the conference secretariat.
If you are unable to attend, the following cancellation fees apply:
Fourteen days before the conference start date: Refund less $50 administration fee, after which refunds will be made at the discretion of the organising committee.
All refunds will be processed following the conference. You may assign your registration to another person but all substitutions must be notified in writing to Six Hats, the conference secretariat.

In the event of industrial disruption or other unforseen circumstances that disrupt the conference, the conference organisers accpet no reponsibility.
The information in this registration brochure is correct at the time of publication. However, the conference organisers reserve the right to change information.
If, for reasons beyond the control of the organising committee, the conference is cancelled, registation fees will be refunded after deduction of expenses already incurred.

Insurance
Delegates are encouraged to take out personal travel and medical insurance which includes loss or damage or personal possessions, including loss of registration and accommodation fees through cancellation.
